Note: The indicative plan of action should not mention actual dates, but should start with "month 1", month 2", etc. Applicants are advised to foresee a security margin in the proposed plan of action. 2.1 ESTIMATED IMPACT ON TARGET GROUPS

Maximum 1 page. Describe the possibilities for replication and extension of project outcomes.
2.4 SUSTAINABILITY
Maximum 3 pages.
A project may be said to be sustainable if it delivers and increases benefits to the target group for an extended period of time after the end of Phare funding. Please justify the measures you intend to take to ensure that your project has a lasting impact and its achievements can be sustained in the long term. If a new structure or organisation is set up in the context of the project, it is particularly important to explain how it will be financed after the end of the project. In this section, also describe the external factors that could contribute to increasing the sustainability of project achievements or, on the contrary, jeopardise them.
Distinguish between the following aspects of sustainability:
(a) financial sustainability (How will the activities be financed after the EC funding ends?)
(b) institutional sustainability (Will structures allowing the activities to continue be in place at the end of the present project? Will there be local "ownership" of project outcomes?)
(c) sustainability at the policy level (where applicable) (What will be the structural impact of the project - e.g. will it lead to improved legislation, codes of conduct, methods, etc?)
3. PROJECT BUDGET
BREAKDOWN OF COSTS
Provide a budget for the total duration of the project. See Guidelines for Applicants, section 2.5 (Payments) for further information.

3.2. JUSTIFICATION OF COSTS
For each budget item in section IV.1, provide a brief justification of costs (max. 5 lines each) and explain the relevance of expenses to project activities. (Please refer to the instructions for the presentation of the budget in the Guidelines).
3.3. EXPECTED SOURCES OF FUNDING
Provide information on the expected sources of funding for the project. Please mention exactly how much each partner, including the Main Applicant (Leader), will contribute to the project, both in finance and in kind. Partners and sponsors (i.e. organisations that make a contribution to project costs but are not partners in the project) to the project may contribute either in finance or in kind. All contributions must be budgeted in EURO. Please ensure that the totals and sub-totals in this section are fully consistent with the amounts indicated in the previous sections. Please ensure that all conditions relating to the sources of funding are met (see Programme Guidelines, . Eligibility of costs: costs which may be taken into consideration for the grant, Applicant(s)' contribution)

Partnership:
A partnership is a relationship of substance between two or more organisations involving shared responsibilities in undertaking the project funded by the Contracting Authority. In order to assist the smooth operation of the project, it is necessary that all partners (including the main applicant which signs the contract) acknowledge this by agreeing to the principles of good partnership practice set out below.
Principles of Good Partnership Practice:
All partners should have read the application form and understood what their role in the project will be.
The applicant should consult regularly with its partners and should keep them fully informed of the progress of the project.
All partners should receive copies of the reports - narrative and financial - made to the Contracting Authority
Substantial changes proposed to the project (e.g. activities, partners, etc) should be agreed by the partners before submitting the proposals to the Contracting Authority. Where no such agreement can be reached, the applicant must indicate this when changes are submitted for approval to the Contracting Authority
Before the end of the project, the partners should agree on an equitable distribution of project equipment, vehicles and supplies purchased with the EU grant among local partners located in the target countries. Copies of the title transfers must be attached to the final report

Additional Annexes
a) Evidence of official registration and statutes
In this annex, please submit the following documents for each of the partners:
Copy of an official document giving evidence that your organisation is formally constituted and registered as a distinct legal entity (registration act, publication in an Official Journal, certificate issued by a competent authority). This official document should notably show the date of registration of the organisation and provide evidence of the non-governmental or the non-profit status of your organisation. In case your organisation is a local branch of a national organisation, please also submit the documents on its foundation.
Submit the statutes of all applicants
Please provide copies of the TAX and VAT registration, as well the documents for identification numbers (if applicable).
Please provide presentation materials such as annual reports and annual accounts, strategic plans, leaflets, etc.
All non-Bulgarian participants are requested to provide their official documents in English. If the document is not in English, please attach an English translation signed and certified by the legal representative of the organisation.
b) Capacity of the team to implement the project
Please attach the CV of the prospective project team members. If some of them have not yet been identified or are to be recruited at the start of the project, please state this explicitly and detail the profile of the person to be recruited.
VI. CHECKLIST
This checklist will help you verify that your project meets all the eligibility conditions and formal requirements of the programme. Your project will be evaluated only if the answer to all questions below is "yes".

GENERAL
r Is your project relevant to the objectives and the activities of the Programme as set out in the Programme Guidelines?

ELIGIBILITY
r Is the Main Applicant (Leader) a nonprofit, non-governmental organisation, registered and based in Bulgaria?
r If you submit a macro-project, does it involve at least one other eligible organisation, formally constituted and based in the Phare candidate countries of Central and Eastern Europe or the European Union and has clear evidence of this been provided with your application?
r Do all partners in the project have a non-governmental or non-profit making status, and has clear evidence of this been provided with your application?
FINANCIAL
r Is the requested EC grant within the limits set out in the Programme Guidelines?
r Minimum 3,000 EURO and no more than 99,999 EURO
r No more than 90% (respectively 80% for projects with EU country involvement) of total costs.
r Is the total amount of the matching contribution supported by written commitments and financed by other sources than EC funds? (Contribution by partners: indicated in their declaration of partnership - Contribution by public or private sponsors: indicated in their declaration of sponsorship)
r Where your budget includes "in kind" expenses, have these been adequately justified, and are they within the authorized limits of no more than 5%, respectively 10% of total project costs?
r Do administrative costs do not exceed 7% of the direct costs?
r Is the budget presented in conformity with the format and instructions of the application form?
