AGREEMENT:
Uniforms are provided complimentary, however, it is my responsibility to provide my own footwear. It is also my responsibility to launder/press my uniforms daily, maintain them immaculately and return them in acceptable condition upon departure. No uniform variations or alterations may be made unless approved by my supervisor.
I have read and understand the uniform standards and dress code agreement outlined in my information packet, and agree to abide by the conditions set forth by the program.
